PLEASE READ AND AGREE TO WAIVER BELOW
In consideration of participating in United Way’s Walk 4 Warmth, on behalf of myself, my heirs, agents, and personal representatives, I hereby understand the nature of the event and assume all risks which might be associated with the event, including risk to bodily injury, property damage, injury to others, and other hazards to me. I further hereby waive, release, discharge, hold harmless, defend and indemnify, and covenant not to sue the United Way of Anchorage, its officers, employees, sponsors, organizers, volunteers or other representatives, and/or their successors, and assigns for any claims, injuries or damages of any kind whatsoever, including claims arising in whole or in part from negligent acts or omissions of the United Way of Anchorage or otherwise, to the fullest extent permitted by law arising out of or relating to participation in the aforementioned event and/or any related activities. I also hereby consent and agree that the United Way of Anchorage may use any photo, film, or videotape of me participating in the event for any lawful purpose.
I also hereby understand that United Way of Anchorage asks for information about me in order to provide information about its programs and events, and request contributions and that United Way of Anchorage will never sell or make available my name or personally identifiable information to third parties.
If registering for a minor; I am a legal guardian of the minor(s) participating as identified above. I certify that I consent to his/her/their participation in the event. I have read the foregoing waiver agreement and I hereby agree on behalf of myself and the minor(s) to all its terms.
