• If you know the Passholder's email address, please provide it. SVPH communicates directly with our Passholders through email regarding upcoming events and Passholder specials. We do not share our email list with other organizations.
• If you are purchasing a Dual or Family Pass, please list the name of all the individuals (aged 5+) you wish to enroll under that pass.  All passholders will be issued membership card.  Passholders will need to present their card at SVPH to receive their benefits.
